Letter To The Patient

Dear Patient:

Thank you for choosing Village SurgiCenter for your upcoming procedure or surgery. Village SurgiCenter and its staff are committed to providing you with the best quality health care available in our region

Effective May 18, 2009, Federal Regulations established an informational program for all patients planning on having a procedure or surgery at an Ambulatory Surgery Center. You or your representative is to be given written information that you need to read and be aware of before your scheduled surgery date. This information includes: Your rights and responsibilities as a patient, information on advance directives, disclosure of physician ownership or financial interest in Village SurgiCenter, and instructions on how to file a grievance or complaint.

These regulations stipulate that you are to be given this information in a language you or your representative can understand. If you desire to receive this information in a language other than English –please contact our office at 814-836-0770. We will send this information to you in the desired language.
Prior to your scheduled surgery date – a nurse from Village SurgiCenter will contact you to complete a pre-operative health assessment. At that time, the nurse will verbally review this information with you or your representative to ensure your understanding. When you arrive at Village SurgiCenter on the day of your scheduled procedure, you will be required to sign a form, indicating that you have received this information prior to your visit and that you understand it.

Village SurgiCenter remains dedicated to the patients in our community and will continue to offer exceptional same-day surgery and procedure care to our patients. If you have any questions or concerns regarding the information you have been provided, please feel free to contact us prior to your surgery date by calling 814-836-0770 Monday through Friday 830 AM through 400 PM. We will be happy to answer your questions.
